Preservice Internship Placement Application

Internship assignments are made cooperatively by college and school district personnel. The information you provide here will assist teacher education faculty in matching you with specific mentors in K-12 schools. We attempt to honor student preferences, but school district needs and availability of mentors take priority. Please be aware that this form, along with your resume, will be sent to school districts for placement purposes.

Instructions

1. Use the following link to download the placement application and open it with Adobe Reader: Internship Application [.PDF]

2. Fill out all fields on the form (this form is a fillable PDF form--you should be able to fill it out on your computer as well as save, print, and submit it via email).

3. Please email completed forms to lcteach@lclark.edu (see below for information on mailing or faxing the application)

Troubleshooting
