Licensing and certification information for mental health counselors and for therapists is available through the Counseling Psychology Career and Professional Resources office (CPSY CPR). Out-of-state institutional verification or licensure request forms should be sent to Sharon Chinn at ECLS, Lewis & Clark, 0615 SW Palatine Hill Road, Portland, OR 97219.

Please note that you must successfully complete all Oregon license/endorsement requirements, including tests, before you can be recommended for an out-of-state license.
